The purpose is to increase data-driven quality of diabetes care and address challenges and opportunities encountered by both Community Health Centers and/or Non-Communicable Disease (NCD) Collaborative teams in the US Pacific.
The goal is to Improve quality of diabetes care provided through clinical data at a local and US Pacific regional level.
